Found the frame with fork, headset, stem and crankset on pignonfixe.com and built it up with stuff i had lying around. The basket is a Wald 139.

Whoever built the frame did nice work on the lugs. Functional and beautiful build, but I find the sort of neon green handgrips sort of jarring with the muted, traditional colors and style of everything else.
