The little metal part is:
Surly's Tuggnuts are beefy chaintugs, well-suited to the requirements of offroaders. Unlike the MKS tugs which we sell to commuters and trackies, the Tuggnut is designed to be used singly. The disk thing is for QR users (instructions enc.) and those projections off the back are bottleopeners.

If you pedal hard, the chain tensioner should prevent the hubs moving forward in the dropouts, preventing the chain from going slack. if you have a half decent wheelset/dropout, chaintugs are pretty useless.
